Identifiable information about you will only be shared in the following circumstances:

  • To provide further medical treatment (e.g. district nurses and hospital services)
  • To help you access other services (e.g. social services) – this requires your consent
  • When we have a duty to others (e.g. safeguarding or child protection cases)
  • Anonymised patient information may be used locally and nationally to help plan services (e.g. diabetic care). This information cannot identify you

If you do not wish anonymous information about you to be used in this way, please contact the practice.

Welsh GP Record

Your Welsh GP Record is a summary of key information from your full GP medical record. It is available to selected healthcare professionals, including Out of Hours services and some hospital providers.

This summary may include:

  • Your name, address and contact details
  • Current medication and prescriptions from the last two years
  • Allergies or adverse reactions
  • Current health problems or diagnoses
  • Results of recent tests (e.g. blood tests or X-rays from the last year)

It does not include private discussions you may have had with your GP.

Management of Records

Records and Information Management are essential parts of Information Governance. They ensure that the right information is available at the right time to support safe and effective decision-making.

We manage records in line with:

  • UK General Data Protection Regulation (UK GDPR)
  • Data Protection Act 2018
  • Freedom of Information Act 2000

Records may include administrative documents, paper and electronic files, emails, audio or video recordings, X-rays, and CCTV footage.
